Invoice Template

Medical Bill Format: Pharmacy and Medical Store Bill Template

A medical bill format for chemists and pharmacies with batch number, expiry date and patient details, a filled sample and a free medical store bill template.

What a pharmacy bill should show

A medical store bill lists each medicine with its batch number and expiry date, so it can be traced if there is a recall. Patients use these bills for insurance and reimbursement, so print the patient name and the prescribing doctor where asked.

GST on medicines depends on the product. Most chemists show a GST-inclusive price on the strip; if you are registered you still show the tax on the bill.
